insert file name into a url from website

Hello-

I am developing a website and am trying to create a form where the user can enter a file name and the form inserts that file name into a url. then when the user hits the submit button they are taken to the coresponding file that they are looking for. this way I can simply add the files to my server with a similar file name format and users can access them easily on their own.
